Patient Referrals

We are always happy to work with you and your patient to ensure that they receive the best care possible. To make the referral process a smooth one, we ask that you fax or e-mail the following to our office at 305-661-1874.

• Patient information including their name, address, phone number, insurance information and Doctor’s name and phone number.
• A diagnosis or reason why the patient requires our care. Also any notes, tests or film reports relevant to the patient’s condition.
• A referral and authorization for the patient visit.

Please note that in addition to the test reports, we will need to see the actual films from your patient’s imaging tests on the day of their appointment.
