Make An Appointment
Our office hours are 9:00 a.m. to 5:00 p.m. Monday through Friday. To schedule an appointment, please call our office at (305) 661-8288 during regular business hours. For your convenience, you may also request an appointment using our Appointment Request Form.

We will contact you by phone to confirm your appointment time once we’ve received your request.
What to Bring
To better serve your needs, we ask that you bring the following items on your first visit:
- If you have had any tests, such as MRI’s, X-rays, CT scans, etc., please bring a copy of the report along with the actual study/films if applicable. You MUST bring the films for the doctor to see you, otherwise your appointment will have to be rescheduled for another time. To obtain films, contact the imaging center where the test was performed or your primary care physician in advance to have your films or compact discs ready.
- A list of all medications you are currently taking.
- A list of any drug and/or food allergies you may have.
- A list of questions you have for the physician.
- Copies of medical records from other doctors related to your visit with us.
- Your health insurance card and information.
- A current picture ID.
Referrals
If your insurance company requires a referral, you must obtain a valid referral from your primary care physician prior to being seen. Please bring the referral with you if our office has not received it in the days before your appointment; otherwise, we will not be authorized to see you.

Cancellations
Please call our office at least 24 hours in advance if you must cancel an appointment so we can open the schedule for other patients that need to be seen.

In case of a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY
